Patient's Bill of Rights & Responsibilities

As a patient at this Hospital/Long Term Care, you have both rights and responsibilities. Both are of great importance to us and we’d like you to fully understand what they include. If you’re the parent or guardian of a child under 18 years of age, you also have rights and responsibilities on behalf of their care.

 Your Rights:

  • Access to available treatment and accommodations regardless of age, sex, marital status, race, creed, color, national origin, source of payment, or the presence of any sensory, mental or physical handicap 
  • Respect and privacy during your care 
  • Confidentiality relative to your care as directed by you or authorized by law 
  • Personal safety 
  • Identification and status of caregivers 
  • Information concerning diagnosis, treatment, and prognosis in terms you can understand 
  • Access to an interpreter if you do not speak English 
  • Access to your medical record and to make corrections if necessary
  • Refusing treatment to the extent permitted by law 
  • Detailed explanation of bill 
  • Information about hospital rules and regulations 
  • Access to people outside the hospital via visitors and written/verbal communications 
  • Timely notice prior to termination of eligibility by third-party payer for cost of care 
  • To be aware of experimental therapy and the right to refuse 
  • The right to participate in the development and implementation of his or her plan of care 
  • To formulate advance directives and to have hospital staff and practitioners comply with directives 
  • The right to have a family member or representative of choice and his/her own personal physician notified promptly of his/her admission to the hospital 
  • To be free from all forms of abuse or harassment 
  • To be free from restraints of any form that aren’t medically necessary

Your Responsibilities:

  • Provide accurate and complete health information to the hospital 
  • Comply with instructions/be responsible for your actions if you fail to do so
  • Fulfill financial obligations
  • Follow hospital rules and regulations
  • Respect the rights of others
  • Ask questions if you do not understand any aspect of your care
  • If you have any questions or concerns regarding your patient rights and responsibilities, please contact your physician, the charge nurse, the Director of Nursing or the Administrator.
